We work in close partnership with you, supporting you to live the life you choose. We take pride in the work we do and in the quality of our tailored services.

Support Coordination

A Support Coordinator will help you to understand and implement your NDIS plan and show you how to best utilize it. Sunbird Coordinators will support you to source and access the best supports and services to address your plan goals, and coordinate all your supports, including informal, mainstream, and funded supports. We encourage you to explore your options and help develop your confidence in connecting with your community. We also work with you and your support network with the aim of building your capacity to undertake the management of your own supports.

Specialist Support Coordination

Specialist Support Coordination includes additional intensive assistance for people with complex needs and high-risk circumstances. Sunbird practitioners are highly qualified and experienced Specialist Support Coordinators who will help you manage the challenges in your environment and reduce barriers to implementing or using your NDIS plan. A Specialist Support Coordinator is designed for people who have a higher level of needs. Some of these needs include: psychological and mental health support, accessing health, education, or justice services, advice and skills to overcome barriers, and ongoing support to reach NDIS goals.

Psychosocial Recovery Coaching

A Recovery Coach provides support to people with psychosocial disabilities to live a full and meaningful life. They work collaboratively with you, your family, carers, and supports to create and implement a Recovery Plan. They support you to take more control of your life with the aim of increasing your participation and independence and will help you to better manage the challenges of day-to-day living. Recovery Coaches also help you understand your NDIS plan and assist with the coordination of other supports while monitoring and reviewing plan goals.
